Introduction
Siddha pharmacology (Materia Medica) is one of the most sophisticated traditional pharmacopoeias in the world, classifying substances into plant, mineral, and animal sources and describing their properties in terms of taste (Suvai), potency (Veerya), post-digestive effect (Vibhaaga), and specific therapeutic action (Prabhava). This article covers five key Siddha herbs that are central to clinical practice but less known outside Tamil traditional medicine, followed by an overview of mineral preparations, classic formulations, and essential safety considerations. For the student, understanding these herbs provides a foundation for the larger pharmacopoeia. For the patient, knowing what their prescribed medicines contain builds trust and compliance. For the clinician, the detailed knowledge of each herb’s properties, preparation methods, and therapeutic range is essential for safe and effective prescribing.
Thuthuvalai (Solanum trilobatum)
Thuthuvalai is one of the most important respiratory medicines in the Siddha pharmacopoeia. The plant is a thorny creeper native to South India, and its leaves, berries, and roots are all used medicinally. According to Siddha texts, Thuthuvalai is hot in potency (Veeryam), pungent and bitter in taste, and acts primarily on Kapam and Vatham. Its classical indication is Kasam (cough) and Swasam (asthma) of all types, but it is specifically indicated for Kapa-type respiratory conditions characterized by excessive mucus, wheezing, and chest congestion.
The berry of Thuthuvalai is considered more potent than the leaf and is used in chronic respiratory conditions including tuberculosis-like presentations described in Siddha texts as Kaya Elai. The classic formulation is Thuthuvalai Lehium, a medicated jam prepared by cooking the leaf juice with sugar, ghee, and spices including ginger, pepper, and long pepper. This preparation is given in doses of 5 to 10 grams daily for chronic respiratory conditions. Modern research published in PubMed-indexed journals has confirmed Thuthuvalai’s anti-inflammatory, bronchodilatory, and immunomodulatory activities, supporting its traditional use in asthma and allergic respiratory disorders.
Vasambu (Acorus calamus)
Vasambu, also known as sweet flag or calamus, is the most important pediatric and neurological herb in Siddha medicine. The rhizome is used after processing (suddhi) to reduce its bitter constituents and enhance its therapeutic properties. Vasambu is pungent, bitter, and astringent in taste, hot in potency, and acts primarily on Kapam and Vatham. It is classified as a Medhya Rasayana (nootropic rejuvenative) in Siddha, used for improving memory, speech development, and cognitive function in children.
The most distinctive Siddha use of Vasambu is in pediatric practice. A small piece of the processed rhizome is tied in a cloth and given to infants to suck or wear as a necklace for teething pain, cough, and digestive disturbances. This practice, documented in Tamil Siddha folklore for centuries, is based on the herb’s antimicrobial and carminative properties. For older children and adults, Vasambu is given in powdered form (125 to 250 mg) with honey for speech delay, memory enhancement, and learning disabilities. It is also a key ingredient in many Siddha neurological formulations for paralysis, epilepsy, and mental disorders. Modern research confirms that Acorus calamus contains alpha-asarone and beta-asarone, which have demonstrated neuroprotective, anticonvulsant, and cognitive-enhancing effects in animal and human studies.
Sirunagai (Alpinia galanga)
Sirunagai, also known as galangal or greater galangal, is the Siddha system’s primary digestive and carminative herb. The rhizome is aromatic, pungent, and warm in potency. It acts on all three humors but is particularly effective for Vatha-Kapa digestive disorders. According to classical Siddha texts, Sirunagai strengthens Agni (digestive fire), removes Ama (toxins) from the gastrointestinal tract, and clears obstruction in the channels (Srothas).
Sirunagai is used in Siddha for dyspepsia, flatulence, loss of appetite, abdominal pain, and inflammatory bowel conditions. It is prepared as Chooranam (powder, 1 to 2 grams for adults), as a decoction for acute digestive complaints, and as an ingredient in compound formulations for chronic digestive disorders. In Tamil Nadu, the fresh or dried rhizome is traditionally chewed after meals as a digestive aid. The rhizome is also used externally as a paste for rheumatism and skin conditions. Modern research has documented Alpinia galanga’s anti-inflammatory, antimicrobial, gastroprotective, and prokinetic properties, which support its traditional digestive indications.
Adathodai (Adhatoda vasica)
Adathodai, known as Malabar nut tree in English and as Vasaka in Ayurveda, is one of the most important respiratory herbs in Siddha. The leaves are the primary medicinal part, though the roots and flowers are also used. Adathodai is bitter, pungent, and astringent in taste, hot in potency, and acts primarily on Kapam and Pittham. Its classical indication is all types of respiratory diseases, particularly those with bleeding tendencies (such as tuberculosis with hemoptysis) and chronic cough.
Siddha preparations of Adathodai include leaf juice (given in doses of 10 to 30 ml for respiratory bleeding and chronic cough), leaf decoction for bronchitis and asthma, and the classic formulation Adathodai Manappagu (a syrup prepared from leaf juice) for pediatric respiratory conditions. The leaves are also smoked (as a cigar or pipe) for asthma relief — a traditional practice known as Pugai. The bronchodilator and expectorant properties of Adathodai are attributed to the alkaloid vasicine, which has been extensively studied and is included in some modern expectorant formulations. Unlike many respiratory herbs, Adathodai can be used safely in Pitha-type respiratory conditions (with fever and inflammation) because of its mild Pitta-pacifying action.
Seenthil (Tinospora cordifolia)
Seenthil, also known as guduchi or heart-leaved moonseed, is the preeminent immunomodulatory herb in Siddha medicine. The stem is the primary part used, though the root and leaves also have therapeutic applications. Seenthil is bitter and astringent in taste, hot in potency, and acts on all three humors — a rare classification that indicates its balanced, Rasayana (rejuvenative) nature. It is classified as a Sarva Roga Nivarana (remedy for all diseases) in classical Siddha texts.
Seenthil is used in Siddha for fevers of all types, immune deficiency, chronic infections, arthritis, diabetes (Madhumegam), and as a general rejuvenative. The most common preparation is Seenthil Kudineer (decoction), prepared by boiling the stem pieces in water until reduced by three-fourths. This is given for 10 to 40 days depending on the condition. Seenthil is also prepared as Chooranam, Lehium, and in combination with other herbs in compound formulations for specific indications. During the COVID-19 pandemic, Seenthil-based formulations were among the Siddha interventions studied for their immunomodulatory and antiviral potential. The immunomodulatory activity of Tinospora cordifolia has been confirmed by multiple modern studies, which have identified polysaccharides, alkaloids, and diterpenoid lactones as the active constituents responsible for its immune-enhancing effects.
Mineral Preparations: Parpam and Chenduram
What distinguishes Siddha from most other traditional medical systems is its sophisticated use of minerals and metals — processed through calcination (Parpam) and sublimation (Chenduram) into bioavailable, non-toxic forms. Parpam are calcined powders prepared by subjecting minerals, metals, or animal products to a series of purification (Suddhi) and incineration (Pudam) processes until they reduce to a fine, light powder that floats on water. Chenduram are red-colored preparations made by grinding the purified substance with herbal juices and subjecting it to controlled heat until it achieves a characteristic red color and desirable properties.
The most famous — and controversial — Siddha mineral preparation is Linga Chenduram, made from red sulfide of mercury (cinnabar). After extensive purification (including treatment with lime juice, cow’s milk, and herbal decoctions) and incineration, the mercury is rendered into a bioavailable form that is given in very small doses (50 to 100 mg) for chronic skin diseases, syphilis, and as a rejuvenative for the nervous system. The Siddha texts specify precise indications, doses, contraindications, and antidotes for mercury-based preparations. Modern chemical analysis has shown that properly prepared Linga Chenduram contains mercury in a bound, non-ionizable form that has limited systemic absorption, unlike the highly toxic ionic mercury compounds used in other contexts. However, improperly prepared Linga Chenduram — or Linga Chenduram administered without proper training — can cause mercury toxicity.
Classic Formulations
Amirtha Kuzhambu is a classic Siddha formulation for diarrheal diseases and dysentery. It contains processed mercury, sulfur, and arsenic combined with herbal ingredients including ginger, pepper, and long pepper. Despite containing toxic raw materials, the processed formulation is considered safe and effective in prescribed doses (65 to 130 mg) for acute diarrheal conditions. The preparation process involves extensive purification and incineration that transforms the toxic minerals into therapeutic compounds.
Kaba Sura Kudineer is a polyherbal decoction widely used in Siddha practice for fevers, respiratory infections, and epidemic diseases. It contains 10 to 15 ingredients including ginger, pepper, long pepper, Coriandrum sativum, Cymbopogon citratus, and Zingiber officinale. During the COVID-19 pandemic, the Ministry of AYUSH recommended Kaba Sura Kudineer as a preventive and supportive intervention. Clinical trials published in peer-reviewed journals have evaluated its safety, immunomodulatory effects, and efficacy as an adjunctive therapy for respiratory infections.
Thirikadugu Choornam is the simplest and most widely used compound Siddha formulation — a three-powder blend of dried ginger (Sukku), black pepper (Milagu), and long pepper (Thippili) in equal proportions. It is taken with honey or warm water for digestive disorders, colds, cough, loss of appetite, and post-digestive toxicity. Every Siddha household in Tamil Nadu has Thirikadugu Choornam in the kitchen, used as both a medicine and a culinary spice. Despite its simplicity, the formulation is considered one of the most important in the Siddha pharmacopoeia because it addresses the root of most disease — impaired Agni — through the synergistic action of three pungent herbs.
Safety Considerations and Quality Standards
The use of mineral and metal preparations in Siddha requires rigorous quality control. The Government of India has established Good Manufacturing Practices (GMP) for Siddha pharmacies, and the Ayurvedic, Siddha and Unani Drugs Technical Advisory Board (ASUDTAB) sets quality standards for raw materials and finished products. The Bureau of Indian Standards (BIS) has published specifications for Siddha medicines. Patients should only use Siddha medicines from pharmacies that display GMP certification and are registered with the state or central drug regulatory authority.
Controversy around heavy metal content in traditional Indian medicines is well-documented. Several studies have found elevated levels of lead, mercury, and arsenic in some commercially available products. However, properly prepared Siddha mineral medicines — prepared according to classical texts with correct purification and incineration methods — contain metals in bioavailable, non-toxic forms that are fundamentally different from raw heavy metals. The classical texts include specific tests for quality: a properly prepared Parpam should float on water, should not revert to metallic form, should be tasteless, and should not cause toxicity when tested in animal models. Siddha clinicians are trained in these quality tests and in the signs of proper versus improper preparation. Patients should always request medicines from GMP-certified pharmacies and report any adverse effects to their practitioner immediately.
