Sanskrit Verse

उपशयः पुनर्हेतुविपरीतविपरीतार्थकारिणां औषधान्नविहाराणाम् उपयोगः सुखवहः।

Explanation

Upashaya and its opposite, Anupashaya, form the fourth part of the Nidana Panchaka. It refers to diagnostic therapeutic trials. Upashaya is the relief of symptoms obtained by using medicines, diet, or lifestyle that are either similar to or opposite to the cause or the disease itself. Anupashaya is the aggravation of symptoms from such trials.

Clinical Importance

This is used in cases of diagnostic confusion. For example, if a joint pain is of uncertain origin, applying a hot poultice (opposite to Vata’s coldness) might give relief (Upashaya), confirming a Vata-dominant diagnosis. If it aggravates the pain, it suggests a Pitta-dominant condition.
